Medical Forms and information
If your child requires medication during the school day then please submit either the online form below or download the paper copy and hand into the school office. Please find important information from the NHS and the Department for Health.
Form 3 is an agreement between parent/carer and school so that medication can administered during the school day. In accordance with school policy, medicine cannot be administered unless this form is completed and submitted by a parent/carer.
